This is a unique opportunity to leverage your skills as a Disability Navigator at NSW Health in Broken Hill, AU.

About the Role

The Disability Navigator will provide expert care coordination and case management for individuals with disabilities who are hospitalized, ensuring they receive necessary supports for successful discharge.

You will collaborate closely with the Specialist Intellectual Disability Health Team (SIDHT) Outreach Clinician for the Far West Local Health District (FWLHD) to support NDIS-eligible individuals who are hospitalized, at risk of hospitalization, or where timely support can expedite access to NDIS services, relieving pressure on FWLHD services.

Key Responsibilities
  • Provide comprehensive care coordination, case management, and navigation for people with disabilities who are in hospital.
  • Support NDIS-eligible individuals who are in hospital, at risk of hospitalization, or where timely support will expedite access to NDIS services.

We employ well over 800 clinicians and support staff across our 9 health facilities in Broken Hill, Balranald, Dareton, Ivanhoe, Menindee, Tibooburra, Wentworth, White Cliffs, and Wilcannia.

Selection Criteria
  • Tertiary qualification and 3 years' clinical experience as an Allied Health Clinician preferably in Social Work or a relevant Allied Health profession, with eligibility for registration with the appropriate professional body.
  • Demonstrated advanced clinical reasoning, experience, and practice skills relevant to assessment and treatment of people with disabilities.
  • Demonstrated experience working in the disability sector, with a strong understanding of navigating the NDIS and alternative disability-related supports.
  • Demonstrated ability to coordinate and communicate complex care plans within a healthcare context, as part of a multi-disciplinary team.
