Discharge Care Coordinator

We are seeking a highly skilled and experienced Discharge Care Coordinator to join our team at Northern Territory PHN. As a key member of our team, you will play a critical role in ensuring seamless coordination of care and services during the discharge process from tertiary and interstate hospitals.
Key Responsibilities- Communicate effectively with hospital staff, including doctors, nurses, and social workers, to gather relevant patient information and discharge plans.
- Act as an advocate for the client's needs and preferences during the discharge planning process.
- Ensure travel arrangements for clients returning to their home communities are appropriate and include necessary equipment such as wheelchairs or escorts.
- Arrange for necessary medical equipment and supplies to be available upon the client's return home.
- Refer patients to relevant support services, such as mental health services, NDIS and social care services as required.
- Participate and contribute to continuous quality improvement within Northern Territory PHN.
- A recognised qualification in Aboriginal Health Practitioner or Registered Nurse with no restrictions on practice.
- Experience in coordinating care, case conferencing and discharge planning, preferably in a remote or Indigenous health context.
- Strong understanding of Aboriginal cultural values and the social determinants of health affecting Aboriginal people in remote communities.
- Excellent interpersonal and communication skills, with the ability to engage effectively with diverse stakeholders, including hospital staff, community health workers, clients, and their families.
