The successful candidate will work collaboratively with the healthcare team to ensure high-quality patient outcomes.
Key Responsibilities:
- Provide sound professional advice and support in the assessment, planning, review, evaluation, and coordination of client care
- Work collaboratively with the healthcare team to ensure high-quality patient outcomes
- Develop and implement effective care plans to meet the needs of clients
- Contribute to the development of policies and procedures to improve patient care
- Participate in quality improvement initiatives to enhance patient outcomes
Requirements:
- Postgraduate qualification in a relevant field
- Experience in a clinical care coordination role
- Strong communication and interpersonal skills
- Ability to work collaboratively as part of a team
- Highly developed problem-solving and analytical skills
Remuneration Package: $154,808 - $161,239 (including salary $132,267 - $139,215)
