Detailed Job Description:

  • Create comprehensive caseload management plans addressing initial referrals, intake, and eventual exits.
  • Cultivate positive tenant outcomes through daily face-to-face client meetings and emergency response.
  • Develop collaborative individual support strategies addressing client needs and goals.
  • Proactively gather feedback from support partners to create additional support strategies.
  • Foster holistic proactive support for clients, including accompanying them to appointments and providing ongoing assistance.
  • Engage in block meetings, resident engagement activities, and new housing sites to raise awareness of support services and referral processes.
  • Maintain and nurture external networks and relationships with community organisations.
  • Closely monitor case plan progress and ensure alignment with client needs and goals.
  • Effectively collaborate with internal departments on complex residents' cases.
  • Promote Support Services and associated programs both internally and externally.
  • Maintain up-to-date documentation within database systems.
