Job Description

The Homeless Support Liaison will oversee a designated caseload, handling everything from initial referrals to intake and exit. They will engage in daily face-to-face client meetings, addressing emergencies promptly and ensuring positive outcomes through follow-up actions.

  • Develop individualized support plans with residents that include interventions and address needs and goals.
  • Proactively seek feedback from relevant partners and follow up weekly to assess client needs and create additional strategies.
  • Deliver proactive, hands-on support to clients, which may include accompanying them to appointments.
  • Participate in block meetings, resident engagement activities, and new housing sites to raise awareness of support services and referral processes among residents.
  • Maintain and nurture networks and relationships with external agencies and community organizations.

The ideal candidate will have a degree qualification in Human Sciences, minimum three years experience in case management, strong knowledge of underlying causes of homelessness and complex needs, flexibility to respond to unplanned client needs, effective communication skills, understanding and respect for cultural diversity, and proficiency in software applications (SRS, Office 365 & Excel).
