DPV Health is seeking an experienced Care Manager to join our team of dedicated professionals. As a Care Manager, you will play a vital role in delivering high-quality care to our clients, working collaboratively with our service teams to develop and implement personalized care plans.

Key Responsibilities:

  • Conduct comprehensive assessments of client needs, developing and implementing care plans that meet their unique requirements.
  • Collaborate with clients, their families, and healthcare professionals to ensure seamless care delivery.
  • Manage client budgets, ensuring effective use of resources and maximizing client outcomes.
  • Develop and maintain strong relationships with community service providers, stakeholders, and partners.
  • Conduct regular reviews of client care plans, making adjustments as needed to ensure optimal outcomes.
  • Ensure compliance with statutory, legal, and contractual requirements.

Requirements:

  • Diploma-level qualification in a relevant field, such as community, aged, or health care.
  • Minimum 1 year of experience in case management, with a strong understanding of community aged care services and consumer-directed care.
  • Proven ability to work with ACAT, RAS, Care Navigators, and other referrers.
  • Strong analytical and problem-solving skills, with excellent customer service and communication skills.
  • Proficiency in Microsoft Office suite and other relevant software.
