Job Description

**Job Title:** NDIS Care Manager - Client Adviser

**Job Summary:** Infinity Supports Care is seeking a highly skilled and experienced NDIS Care Manager to join our team. As a Client Adviser, you will play a critical role in providing independent, continuous advice and guidance to our clients across Melbourne.

**Key Responsibilities:**

  • Review intake information, engage new clients, establish home care agreements, craft care plans, budgets, and connect clients with services and supports.
  • Conduct mobile work, visiting clients as required, implementing person-centered care planning, and providing independent advice and guidance to clients and their families.
  • Collaborate with clients, caregivers, and representatives to ensure their ongoing needs and preferences are supported for independent living at home.
  • Liaise with Infinity Supports Care employees, stakeholders, and partnered service providers on behalf of the client and Infinity Supports Care.
  • Conduct annual reviews for all clients, revising care plans, goals, services, and budget expenditure.
  • Ensure ethical, fair operations within statutory, legal, and contractual requirements.
  • Attend Head Office for training and team meetings.

**Qualifications:**

  • Tertiary qualifications in allied health, social science, health/welfare discipline, or a related field.
  • In the absence of completed tertiary qualifications, experience in the delivery of person-centered practices in non-complex and/or complex case management, and/or demonstrated experience with complex service-oriented offerings in Disability services.

**Additional Requirements:**

  • All appointments are contingent upon a satisfactory National Police Check conducted by Infinity Supports Care and a valid Working with Children Check provided by the applicant.
  • A statutory declaration confirming residency in a country other than Australia is required. Depending on the information provided, international police checks may also be required.
  • Part of our selection process involves completing psychometric assessments.
