Job Title

Requirements
">- A bachelor's degree in a relevant allied health discipline
- Experience working with clients with chronic and complex health needs
- Strong communication and interpersonal skills

About the Role
">This position plays a key role in linking patients into services appropriate to their individual needs. The team aims to improve continuity of care for patients in public correctional centres with Drug and Alcohol problems who are being released to the community.
The Connections Program uses an assertive linkage model to connect patients with relevant health and welfare service providers pre- and post-release.
