Job Title: Complex Care Specialist
We are seeking a skilled and experienced Complex Care Specialist to join our team. This is an exciting opportunity for a Registered Nurse with a passion for mental health care to make a difference in the lives of older adults.
The successful candidate will provide high-quality care coordination for individuals with complex mental health needs, working across multiple sites. This role requires a strong ability to assess, plan, implement, and evaluate interventions and care plans, as well as extensive experience in conducting comprehensive mental health assessments.
Key Responsibilities:
• Provide care coordination for older adults with complex mental health needs
• Conduct comprehensive mental health assessments and develop individualized care plans
• Implement and evaluate interventions and care plans, ensuring optimal outcomes for clients
• Work collaboratively with internal and external partners, including Residential Aged Care Facility staff
Requirements:
• Registered Nurse with a minimum of 5 years post-graduate experience in mental health specialty
• Approved post-graduate qualification within mental health, drug and alcohol, or other qualifications deemed appropriate
• Demonstrated experience working with consumers and carers presenting with complex mental health needs
