Complex Mental Health Specialist

Seeking an experienced Registered Nurse to join our community team as a Clinical Nurse Consultant Complex Care.
This role involves providing care coordination for older people with complex mental health issues, including physical co-morbidities and psycho-social challenges.
You will provide education and guidance to internal and external partners, including Residential Aged Care Facility staff.
The successful candidate will have a minimum of 5 years full-time post-graduate experience in mental health, along with an approved post-graduate qualification within mental health, drug and alcohol, or other relevant qualifications/experience.
Key responsibilities include conducting comprehensive mental health assessments, developing and implementing consumer-focused care plans based on evidence-based practice.

- Provide care coordination for older people with complex mental health needs

Candidates Will Need To Meet The Following Criteria
- Registered Nurse with a minimum of 5 years full-time post-graduate experience in mental health specialty
- Demonstrated experience working with consumers and carers presenting with complex mental health needs
- Ability to assess, plan, implement and evaluate interventions and care plans
