Complex Mental Health Care Specialist

Overview:
Join a dynamic multi-disciplinary community team as a Clinical Nurse Consultant in Complex Care. In this role, you will provide care coordination for older people who present with complex mental health issues.
Job Description:
- Provide comprehensive mental health assessments and develop, implement, and evaluate consumer-focused care plans based on evidence-based practice.
- Collaborate with internal and external partners to provide education and guidance on mental health and wellbeing.
- Work with consumers and carers presenting with complex mental health needs and/or behaviours of concern.
Required Skills and Qualifications:
- Registered Nurse with a minimum of 5 years full-time post-graduate experience in a mental health specialty.
- Approved post-graduate qualification within mental health, drug and alcohol, or other qualifications/experience deemed appropriate.
