About the Role

We are seeking a dedicated and compassionate individual to join our Transition to Adulthood team as a Support Practitioner. In this role, you will work closely with young people transitioning out of home care, providing guidance and support to help them achieve positive outcomes in adulthood.

Key Responsibilities

  • Deliver coaching, care coordination, and advocacy services to young people transitioning from out-of-home care
  • Collaborate with key stakeholders to create a positive experience for young people
  • Mentor young people to help them set and achieve personal goals
  • Provide practical support in housing, employment, health, and overall wellbeing

Requirements

  • Degree in Social Work, Psychology, Social Sciences, or a related discipline
  • Minimum of two years' experience in a human services role
  • Solid understanding of human services and care coordination
  • Knowledge of the Child Protection Act 1999
