About the Role
We are seeking a dedicated and compassionate Transition to Adulthood Support Practitioner to join our team at Churches of Christ, Children Youth and Families in Mount Isa. As a key member of our Transition to Adulthood team, you will play a vital role in supporting young people transitioning from out-of-home care to adulthood.
About the Team
Our Transition to Adulthood team is committed to ensuring young people are prepared to achieve positive, long-term outcomes as they move into adulthood. We work collaboratively with a wide range of stakeholders to help young people develop skills, access resources, and plan for their future.
Your Key Responsibilities
- Deliver effective coaching, care coordination, and advocacy for young people transitioning from out-of-home care.
- Collaborate with key stakeholders, including natural mentors, Child Safety, health providers, and community support services, to create a positive experience for young people.
- Mentor young people to help them set and achieve personal goals, finding creative solutions to overcome barriers.
- Provide practical support in housing, employment, health (including mental health), and overall wellbeing to establish a foundation for growth.
- Work sensitively with Aboriginal and Torres Strait Islander people and communities to ensure culturally inclusive services in a safe environment.
Requirements
- A degree in Social Work, Psychology, Social Sciences, or a related discipline.
- A minimum of two years' experience in a human services role, ideally within a multidisciplinary team.
- A solid understanding of human services and care coordination.
- Knowledge of the Child Protection Act 1999, or a willingness to learn.
- Experience in trauma-informed care principles, particularly in assessment, planning, and decision-making for young people.
