
Aboriginal Healthcare Navigator

This is a part-time, permanent role located at the Caringbah Community Health Centre. As a clinician on the OOHC Health Pathway Program, you will be part of a multidisciplinary team providing quality assessment, review, and care coordination services to clients.
Key Responsibilities:- Patient Assessment and Care Coordination: Provide comprehensive assessments and develop individualized care plans to support children, young people, and their families in accessing healthcare services and improving health outcomes.
- Cultural Connections: Build and maintain relationships with Aboriginal Community Controlled Organisations, community services, and the local Aboriginal community to ensure culturally sensitive care delivery.
- Interdisciplinary Collaboration: Work closely with the multidisciplinary team to provide holistic care to clients on the OOHC Health Pathway Program.
- Qualifications and Experience: Possess qualifications in Social Work, Psychology, Occupational Therapy, or Counselling, and have demonstrated core competency skills in culturally informed assessment and care coordination within an Out of Home Care or trauma setting.
- Skills and Competencies: Develop high-level written, verbal, and interpersonal skills to engage clients and work effectively with the team.
- Cultural Awareness and Commitment: Demonstrate understanding of challenges experienced by Aboriginal children and young people in accessing healthcare services, and commit to clinical and cultural supervision and professional education.
This is an identified Aboriginal position, and applicants must provide documentation to confirm their Aboriginality as part of the recruitment process. The successful applicant will be required to demonstrate a commitment to working with Aboriginal communities and promoting reconciliation.
