Support Coordinator Role

We are seeking a Support Coordinator to provide individualised support to participants with complex disabilities in the National Disability Insurance Scheme (NDIS). The successful candidate will work directly with participants to implement supports within their plan and build capacity, manage the impact of their disability, and improve their health, wellbeing and participation in the community.

Key Responsibilities:
  • Provide high-quality support planning and coordination services to participants.
  • Work collaboratively with participants, their families and carers, and other service providers to achieve positive outcomes.
  • Develop and maintain effective relationships with participants, their networks, and other stakeholders.
  • Conduct thorough needs assessments and develop tailored support plans that meet the unique needs of each participant.
  • Monitor and evaluate the effectiveness of support plans and make adjustments as needed.
Required Skills and Qualifications:
  • Certificate IV in Disability or Mental Health or other qualification relevant to the field of human services.
  • Previous experience working with people with a disability or mental health diagnosis.
  • Previous experience in support planning, working within a person-centred framework.
  • Sound organisational and administrative ability.
  • Current NDIS Worker Screening Check or willingness to obtain.
  • Current Working with Children Check or willingness to obtain.
  • Fully vaccinated against COVID-19, unless you are an Excepted Person as defined by the COVID-19 Mandatory Vaccination (Workers) Directions.

Senior Support Coordinator Role

We are also seeking a Senior Support Coordinator to provide high-level and complex plan implementation support to participants with complex disabilities in the NDIS. The successful candidate will focus on addressing barriers and reducing complexity in the support environment, while assisting the participant to connect with supports and build resilience.

  • Higher level qualification, including Psychologist, Occupational Therapist, Social Worker, other allied health, developmental educator, social or health science profession.
  • Proven experience working with people with disability and/or mental illness. Experience in the NDIS environment is desirable.
  • Demonstrated ability to network with local service providers.
  • Demonstrated experience in support planning, working within a person-centred framework.
  • Well-developed interpersonal and communication skills, both written and verbal.
  • Sound organisational and administrative ability including reporting outcomes to senior management.
  • Current NDIS Worker Screening Check or willingness to obtain.
  • Current Working with Children Check or willingness to obtain.
  • Fully vaccinated against COVID-19, unless you are an Excepted Person as defined by the COVID-19 Mandatory Vaccination (Workers) Directions.
