About the Role
We are currently seeking a clinical professional to provide in-home nursing services, conducting assessments and collaborating with our care teams to ensure holistic, person-centred care.
- Deliver high-quality clinical services such as wound care, medication management, catheter care, and chronic disease support
- Conduct comprehensive health assessments and reviews
- Develop and monitor individualized care plans in collaboration with clients, families, and coordinators
- Provide clinical oversight and mentoring to care staff
- Maintain accurate and timely documentation and adhere to relevant compliance and safety standards
Requirements
- AHPRA registration as a Registered Nurse (Level 1)
- Experience in aged care, disability support, or community nursing
- Strong clinical judgment and problem-solving abilities
- Excellent communication and interpersonal skills
- Valid driver's license
- Current Police Check, CPR/First Aid, and up-to-date COVID-19 vaccination (or willingness to obtain)
