Purpose of Position

The Clinical Care Partner collaborates with participants, their families, registered supporters, formal and informal supports, and other service providers to facilitate consumer-directed aged care support and services tailored to achieving each participant's goals. This role focuses on assisting older individuals in maintaining their independence at home, ensuring alignment with CÜRA Community Services Mission, Vision and Values, legislative guidelines and funding agreements.

Utilising strong clinical and relationship building skills, and in partnership with the participant, this role will provide direct and indirect nursing supports to assist participants to achieve and maintain their wellbeing goals. The role will provide guidance and support to non-clinical Care Partners, ensuring oversight of Clinical Governance implementation and the delivery of clinical care and support that is safe, effective, and tailored to the needs of the individual.

Responsibilities:

  • Delivery of specialised services to maintain or regain functional and/or cognitive capabilities, aligning with the Clinical Governance Framework across the Cura Aged Care program.
  • Delivery of best practice, evidence-based prevention, diagnosis, treatment and management practices to deliver safe and quality care to the older people accessing Cura services.
  • Direct management of a case load of participants with complex clinical needs as determined by the Cura Service Manager, developing comprehensive and innovative Care Plans within individual budgets, in consultation with participants and all key stakeholders.
  • Conducting of clinical assessments and prescribing of nursing related consumables within the Registered Nurse scope of practice and within the guidelines of the relevant program.
  • Development of Care Plans that provide evidence-based instruction, support and oversight to the Enrolled Nurse team members in the delivery of direct clinical services within their scope of practice.
  • Liaising with third party stakeholders such as Allied Health professionals, GP's and Hospital Discharge teams.
  • Regular liaising and reporting of participant requirements with GP's, by phone, email or using medical objects.
  • Providing of clinical oversight, support and advice to non-clinical members of the Cura Care Partner Team, including the endorsing of participant Care Plans.
  • Participation in multi-disciplinary case conferencing to review and re-negotiate Care Plans, respecting each participants right to dignity and self-determination.
  • Provision of training within scope of practice to office and field staff as required.
  • Ability to work with a diverse range CALD participants, including the use of interpreters as required.
  • Provision of prompt follow up and home visiting when participant deterioration or hospital discharge is identified.
  • Providing input into Cura organisational Clinical Governance processes and active participation in the Cura Quality Care Advisory Body (QCAB).
  • In accordance with Accreditation and Quality Framework, provide input into the development and monitoring of appropriate quality controls, prevention of risk, and maintaining of safe working practices across the organisation.
  • Perform other duties as requested to meet changing organisational needs.
  • Behave in a manner that is consistent with the organisations Code of Conduct, to ensure all interactions with stakeholders are positive and represent the values of the organisation.
  • Lead by example as a clinical mentor and member of the clinical team, providing peer support in the spirit of working together.
  • Provide direction and support to Cura team members around clinical service delivery.
  • Provide direction and support in identifying and managing participant deterioration.
  • Positive leadership and people skills, including the ability to effectively coach, mentor and collaborate with the Care Partner and wider Cura Team.
  • Build relationships within the Aged Care Sector and wider community to build on service delivery outputs.
  • Participate in team building activities to ensure Cura Aged Care values are consistently demonstrated.
  • Promote and develop Cura Aged Care networks and partnerships, building strong services linkages.
  • Ensure the efficient use of program resources within individual participant budgets.
  • Attend meetings, professional development and consultation forums as required.

Desired Skills and Experience:

  • Must hold a Degree in Nursing
  • Maintain currency around APHRA registration.
  • Knowledge and experience in implementing Industry best practice.
  • A minimum of three (3) years' experience in a similar position
  • Must hold a current Driver's License
  • Hold a current National Police Certificate or has the ability to obtain one
  • Demonstrated experience in the community sector
  • Demonstrated experience and understanding of the guiding principles of Consumer Directed Care
  • Excellent written and oral communications skills, including strong presentation, negotiation and liaison skills.
  • Demonstrated ability to coordinate a team
  • Ability to develop and manage individual budgets with customers and their stakeholders.
  • A demonstrated ability to identify and develop solutions to complex issues proactively.
