Job Description:We are seeking an experienced and compassionate Clinical Care Specialist to join our team in a 24-bed acute medical unit at a private hospital. As a key member of the multidisciplinary team, you will play a vital role in planning, assessing, delivering, and evaluating patient care.
The successful candidate will have a strong background in acute hospital experience and be able to work effectively in a collaborative environment. You will be responsible for providing high-quality patient care, developing and implementing individualized care plans, and working closely with other healthcare professionals to achieve optimal patient outcomes.
- Key Responsibilities:
- Assessing patients' needs and developing comprehensive care plans.
- Providing high-quality patient care and support.
- Collaborating with the multidisciplinary team to achieve optimal patient outcomes.
- Maintaining accurate records and contributing to quality improvement initiatives.
To be considered for this role, you must have:
- Current registration with AHPRA as an Enrolled Nurse (IV Qualified).
- Minimum of 2 years' acute hospital experience.
- Flexibility to work a 7-day rotating roster.
- Strong time management, critical thinking, and organisational skills.
