We are seeking a highly skilled Community Health Nurse Specialist to join our team in the transition aged care program.
About the Role- This position aims to improve the independence of individuals receiving transitional aged care, providing home-based rehabilitation for up to 12 weeks.
- Conduct comprehensive assessments in clients' homes to identify their healthcare needs and develop individualized care plans.
- Liaise with multidisciplinary teams to ensure seamless care coordination and delivery.
- Provide nursing interventions and education to clients and their families to enhance health outcomes.
- Monitor and review clients' progress, making adjustments to care plans as necessary.
