Job Title: Patient Transition Coordinator

Job Description:

A Discharge Planner plays a vital role in ensuring patients receive the necessary care and support after hospital discharge. This is particularly crucial for rehabilitation patients who require continued assistance to recover.

The successful applicant will be responsible for coordinating the discharge process of all Rehabilitation patients, working closely with hospital teams to ensure a smooth transition.

Key Responsibilities:

  • Coordinating and planning effective patient discharges to suitable accommodation or their usual residence.
  • Developing a clinical approach to quality patient care.
  • Providing education and support to patients and their families through collaboration with hospital teams.
  • Monitoring and evaluating the effectiveness of the discharge program.

Required Skills and Qualifications:

  • Bachelor's Degree in Nursing.
  • Full registration with AHPRA.
  • At least 5 years' experience as a senior acute care nurse.
  • Proven ability to implement programs and identify gaps within the role.
  • Excellent customer service and interpersonal skills.
  • Demonstrated communication skills.
  • Excellent IT skills.
  • Excellent communication and negotiation skills.
