Job Opportunity

As a Transition Coordinator, you will play a key role in ensuring seamless transitions between disability support and medical care.

We are a dynamic organisation with an experienced team of support workers and nurses who are passionate about delivering exceptional client outcomes.

Our unique clinically led company has seen significant growth year on year, making us an exciting place to work.

Our office team is supportive and collaborative, with regular social activities every Friday and bigger team events monthly.

In this project management focused role, you will work closely with internal stakeholders including doctors, nurses, and risk teams, as well as external stakeholders such as clients, support coordinators, allied health professionals, and hospitals.

You will be responsible for coordinating client profiles, intake assessments, financial assessments, and service agreements, as well as managing expectations around accommodation readiness.

This is a new role created due to the need for a key person to transition clients from external settings to their new home. You will be the first point of contact and will coordinate all areas of the transition to ensure smooth and timely delivery.
