Care Transition Specialist

The Continuing Care Coordinator is a pivotal role in the Patient Flow Management team at Nepean Hospital. As a clinical expert, this position facilitates seamless care transitions for patients referred through e-Orders.
This challenging yet rewarding role requires strong communication and coordination skills to ensure timely facilitation of patient transfers. The ideal candidate will be able to lead evidence-based practice and embody the CORE values of Collaboration, Openness, Respect, and Empowerment.
As part of the multidisciplinary healthcare team, the Continuing Care Coordinator will work collaboratively with other professionals to provide high-quality patient care.
Key aspects of this role include:
- Clinical Expertise: Provide specialized nursing consultancy services to patients, carers, and other healthcare professionals.
- Coordination and Planning: Facilitate timely coordination and planning of safe transfers of care for patients referred through e-Orders.
- Communication: Demonstrate effective clinical communication using information technology.
