Clinical Nurse Specialist

We are seeking a highly skilled Clinical Nurse Specialist to join our team at Australian Unity, working in partnership with Ramsay Connect. This is a fixed-term contract opportunity for 12 months, with the possibility of extension.
Key Responsibilities- Deliver high-quality, client-centered care to individuals accessing the Ramsay Connect Virtual (RCV) Heart Failure service.
- Lead the integration of care pathways in the South East Queensland area, working closely with hospital and primary care teams.
- Establish strong working relationships with specialists, hospitals, and GPs in the area to grow referral pathways into the service.
- Foster integration with the service through prompt escalation, case conferences, and medication titration.
- Advanced clinical skills and recent cardiac experience, with a focus on heart failure management.
- Ability to work within a multi-disciplinary framework, delivering care that is responsive to individual needs.
- Contribution to continuous improvement and service development activities, ensuring the highest standards of care.
- Experience working with multiple stakeholders to provide appropriate engagement and education.
- Full clean driver's license and ability to commute to hospitals in the Brisbane, Gold Coast, and Sunshine Coast areas.
- Tech-savvy, with strong administration and time management skills.
- Strong work ethic and ability to deliver high-quality work within short time constraints.
- Registered Nurse with unrestricted AHPRA registration.
