Expert Patient Care Nursing Professional

Geisinger Home Health is seeking a skilled Home Health Nurse Care Manager to join our team. As a key member of our healthcare team, you will provide expert nursing care to patients in their homes.
- Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
- Receive referrals, assign clinicians appropriately, evaluate patient orders, and initiate start of care visits.
- Coordinate determination of patient home health benefits, medical necessity, and insurance approvals.
- Assess patient needs continually and ensure care is individualized, appropriate, and meets eligibility criteria and physician orders.
Our ideal candidate has current RN licensure in the state of practice, current CPR certification, and valid driver's license, vehicle insurance, and access to reliable transportation.
