Patient Care Nursing Professional

This is a role that focuses on providing and directing nursing care to patients in their homes as prescribed by the physician.
- The Home Health RN Patient Care Manager supervises and coordinates clinical services, ensuring that patient care meets eligibility criteria and physician orders.
Key responsibilities include:
- Providing clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with the healthcare team.
- Receiving referrals, assigning clinicians appropriately, evaluating patient orders, and initiating start of care visits.
- Coordinating determination of patient home health benefits, medical necessity, and insurance approvals.
- Assessing patient needs continually and ensuring care is individualized, appropriate, and meets eligibility criteria and physician orders.
For this role, we require:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to reliable transportation.
