Job Description

We are seeking a highly skilled Key Worker to join our team. As a Key Worker, you will play a vital role in delivering early childhood intervention services to support children with developmental delays or disabilities, and their families.

This position is designed to deliver supports under a Key Worker model, where you will work closely with our team to help families overcome barriers and unlock their child's full potential.

Key Responsibilities
  • Planning and Coordinating Supports: Develop and implement personalized plans to support children and families, reflecting strengths-based and family-centered approaches.
  • Delivery of Supports: Provide high-quality supports to children and families, utilizing a transdisciplinary Key Worker approach to achieve positive outcomes.
  • Assessment and Communication: Regularly assess and communicate the child's development and progress, ensuring that parents and caregivers are informed and engaged.
  • Team Facilitation: Build and facilitate a collaborative team around the child, promoting participation in family, community, and cultural activities.
  • Documentation and Coordination: Ensure that all supports are clearly documented, coordinated, and communicated to families and stakeholders.
