Care Delivery Specialist

We are seeking a highly skilled Occupational Therapist to join our multidisciplinary team in delivering exceptional care to patients with complex and chronic health conditions.
- The successful candidate will be responsible for conducting comprehensive needs assessments, developing tailored treatment plans, and collaborating with internal and external healthcare providers to ensure seamless patient care.
- Key responsibilities include providing expert consultancy, education, and support to individuals, families, allied health professionals, and nursing staff to enhance patient outcomes.
Our ideal candidate possesses extensive experience working with consumers who have chronic and complex illnesses, including expertise in advance care planning discussions.
Key Selection Criteria include:
- Current Registration as an Occupational Therapist with the relevant professional body or eligibility for accreditation as an Accredited Exercise Physiologist.
- Proven experience in care planning and case management of community-based patients with chronic and complex illnesses.
- Strong understanding of specialist palliative care, palliative approach, and community health services.
