
Chronic Care Specialist

The role of Occupational Therapist is to provide care planning and advance care planning for patients with late-stage chronic and complex health conditions.
Candidates will be responsible for conducting palliative care needs assessments and developing collaborative treatment care plans.
Additional responsibilities include making referrals to relevant internal and external health and community support providers, offering education and support to other clinicians to support complex patient needs.
Key Responsibilities:- Conduct comprehensive assessments to identify patient needs and develop personalized care plans.
- Provide expert advice and guidance on occupational therapy practices and interventions.
- Collaborate with multidisciplinary teams to ensure seamless care delivery.
To be successful in this role, candidates must possess current registration as an Occupational Therapist with AHPRA or eligibility for accreditation as an Accredited Exercise Physiologist with ESSA.
Candidates should have extensive knowledge and experience working with consumers who have chronic and complex illnesses, including ability to conduct advance care planning discussions.
Experience in care planning and case management of community-based patients with chronic and complex illnesses is also essential.
Desirable Skills and Qualifications:- High level interpersonal and communication skills (written, verbal, computer) and ability to work collaboratively in a multidisciplinary team.
- Clinical leadership capabilities with ability to provide consultancy, education and support to individuals, families, allied health and nursing staff, and other health professionals.
