Family Wellbeing Service Practitioner

About the Role:
  • The Family Wellbeing Service Practitioner will work in partnership with families to empower them and support them in overcoming identified family challenges.
  • The role involves working in a community-based setting, engaging with families in their homes and developing strong relationships to promote positive change.
Key Responsibilities:
  • To develop and implement individualised case management plans to meet the needs of clients.
  • To provide high-quality and culturally responsive practice to clients from diverse backgrounds.
  • To collaborate with internal and external stakeholders to ensure seamless service delivery.
  • To participate in regular team meetings and contribute to continuous quality improvement.
Requirements:
  • A current bachelor's degree in Social Work, Human Services, or a related field.
  • Minimum 2 years' experience in a similar role or relevant sector.
  • Strong communication and interpersonal skills.
  • Ability to work in a performance-driven environment.
