About the RoleThe Family Wellbeing Service Practitioner will work closely with families to build trust and develop strong partnerships. This involves conducting home visits, identifying family needs, and delivering interventions that promote self-awareness and empowerment. You will be responsible for meeting individual case performance targets, providing high-quality practice, and maintaining accurate records.
You will have the opportunity to work with diverse clients, developing a deep understanding of their unique challenges and strengths. Your expertise will be invaluable in helping families navigate complex issues and achieve their goals.
We value your commitment to providing culturally sensitive and responsive care. Your knowledge and skills will enable you to effectively engage with clients, foster positive relationships, and deliver outcomes-driven solutions.
Key Responsibilities- Meet individual case performance targets as per weekly, monthly, and quarterly dashboard system
- Provide high-quality practice in all interactions with clients, consistent with our values and vision
- Deliver targeted interventions and facilitate client-led assessments and care plans
- Cultivate productive working relationships at all levels, supporting internal and external collaboration
To succeed in this role, you will need:
- A current bachelor's degree in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, or Criminology
- At least 2 years' relevant experience in social services, including case management and behaviour change interventions
- Demonstrated ability to work in a performance-driven environment, meeting individual targets and deadlines
