Family Wellbeing Service Practitioner

This role involves collaborating with families to empower them and help them build agency and overcome identified challenges.

About the Position:
  • The primary focus is on outreach, engaging families in their homes and building strong ongoing relationships.
  • Families are not assessed for issues by practitioners but instead receive interventions that help them develop awareness and insight into their history and current challenges, enabling them to identify their key challenges and lead their own change.
Main Responsibilities:
  1. Maintain individual case performance targets as per weekly, monthly, and quarterly dashboards.
  2. Provide high-quality practice in all interactions with clients, aligned with organisational values and vision.
  3. Facilitate a client-led assessment and care plan to identify client goals, including the entire family where appropriate.
  4. Offer targeted interventions and family-facilitated group meetings to achieve client objectives.
  5. Nurture productive working relationships to support internal and external collaboration, respect diversity, and guide staff development.
Requirements:
  • A current bachelor's degree in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention, or Criminology, along with 2 years' relevant experience in social services; OR
  • A current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, or a related field, with 3 years' relevant experience and at least 1 year in a case management role.
  • An ability to work effectively in a performance-driven environment to meet targets.
