Family Wellbeing Service Practitioner Role Summary:

This position is part of a comprehensive service aimed at empowering families to overcome challenges and achieve their goals. The Family Wellbeing Service Practitioner will work collaboratively with families to build agency, self-efficacy, and facilitate change through client empowerment.

Key Responsibilities:
  • Meet individual case performance targets as per weekly, monthly, and quarterly dashboard system.
  • Provide high-quality, intent-based practice in all interactions with clients, consistent with organisational values and vision.
  • Deliver interventions that build family awareness and insight into their history and current family challenges.
  • Facilitate client-led assessments and care plans to identify client goals, including the entire family where appropriate.
  • Provide role-specific targeted interventions and family-facilitated group meetings to achieve client goals.
Required Skills and Qualifications:
  • Current bachelor's degree in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention, or Criminology.
  • 2 years' relevant experience in social services field.
  • Demonstrated ability to work in a performance-driven environment to meet individual weekly, monthly, and quarterly performance targets.
  • Ability to build agency, self-efficacy, and facilitate change through client empowerment.
Benefits:
  • Multiple permanent full-time positions with a starting annual salary of $93,613 (dependent on qualifications and experience).

Additional Requirements:
  • Requirement to travel across the Logan and/or Brisbane region, as per program location.
  • Current drivers' licence.
  • Valid police check.
  • Flexibility to work outside core 08:30am - 5:00pm business hours in meeting individual client needs.
