Key Responsibilities- Meet individual case performance targets by delivering high-quality services that meet the needs of clients.
- Provide practice-based evidence that aligns with organisational values and vision.
- Utilise effective case management practices and record keeping systems to ensure operational requirements are met.
- Engage clients through assertive outreach approaches, facilitating strong partnerships and achieving client goals.
- Facilitate assessments and Care Plans to identify client goals, including the entire family where appropriate.
- Deliver targeted interventions and family facilitated group meetings to achieve client outcomes.
- Commit to quality management through the use of LogiQC systems and MobNet tasks.
- Cultivate productive working relationships, guiding staff development and mentoring for growth.
