Family Support Worker


About the Role

We are seeking a skilled and experienced Family Support Worker to join our team. As a key member of our team, you will be responsible for delivering strength-based case management and activities to children, young people and families.

You will work closely with participants, community members and other services to build relationships and communicate effectively.

Your expertise in working with Indigenous and CALD communities will be invaluable in this role.


Responsibilities
  1. Deliver strength-based case management and activities to children, young people and families.
  2. Provide outreach support to assist in the development and implementation of participant-centred plans.
  3. Establish referral pathways into and out of the service to reach vulnerable populations.
  4. Provide education, training and information to promote awareness of mental health.
  5. Support other programs as needed.

Requirements
  • Diploma or Certificate level qualification in a relevant health or welfare discipline.
  • Demonstrated experience in working with children, young people and families.
  • Understanding and application of a strength-based approach.
  • Ability to build relationships and communicate effectively.
