Job Title: Clinical Care Coordinator

The role of the Clinical Care Coordinator plays a vital part in supporting older Australians to live safely and comfortably within their own homes.

Key Responsibilities:
  • Deliver clinical and personal care duties with a holistic approach to in-home patient care, focusing on individual needs and requirements.
  • Utilize expertise and judgment to develop comprehensive care plans aligned with assessed client needs, incorporating input from multidisciplinary teams where necessary.
  • Regularly monitor chronic and palliative conditions, adapting care strategies as needed to ensure optimal outcomes.
  • Evaluate wounds and attend to dressings according to established care plans, adhering to best practice guidelines.
  • Assist clients in utilizing continence devices and facilitate changes to IDC and SPC as required.
  • Provide support with showering, bathing, shaving, dressing, toileting, denture care, and other essential personal care activities.
  • Prioritize the delivery of holistic care encompassing social, emotional, and spiritual support, acknowledging the unique needs and preferences of each client.
Requirements:
  • Holding unrestricted AHPRA registration, with a minimum of 2 years' experience practicing as an Enrolled Nurse (EN).
  • Desirable qualifications include prior experience working in Aged Care and Community Home Care settings, along with excellent customer service skills.
  • Maintaining current First Aid certification (HLTAID003/4) and CPR certification (HLTAID001) is also essential.
  • The successful applicant must hold the right to work in Australia and possess a valid driver's license, vehicle registration, and access to a car for travel purposes.
  • National Police Clearance or willingness to obtain clearance through WorkPro at no additional cost to the employer is mandatory.
  • A Pre-Employment Medical assessment will be required prior to commencement of employment.
