This role presents an exceptional opportunity to lead the implementation of transformational healthcare initiatives as part of our district's Connected Care Service. Collaborating with dynamic teams across the district, you will shape the future of healthcare delivery by driving innovation and leading change.

As a seasoned health service planner and project manager, you will possess expertise in stakeholder engagement, clinical service development, and change management. With your exceptional interpersonal skills, you will inspire and influence diverse stakeholders to implement new virtual clinical pathways and drive transformative change.

Developing innovative models of care that enhance clinical and operational efficiencies is at the heart of this role. Strong communication skills, adaptability, and resilience are essential personal attributes for success in this position.

The ideal candidate will evaluate current services, identify opportunities for innovation, develop new virtual clinical pathways, and lead change management initiatives. With a focus on remote patient monitoring services, this role requires extensive collaboration with clinical and non-clinical staff, patients, consumers, senior leaders, and external partners to align our services with community needs.

Key Responsibilities:
  • Lead the development and implementation of innovation projects aligned with our Transforming Your Experience strategy.
  • Evaluate current services and identify opportunities for innovation.
  • Develop new virtual clinical pathways and lead change management initiatives.
  • Collaborate with clinical and non-clinical staff, patients, consumers, senior leaders, and external partners.
Requirements:
  • Relevant tertiary qualification in a health management field or relevant experience in a health-related or health management field.
  • Demonstrated experience in project management, ensuring coordination of diverse inputs and achievement of optimal performance within an environment of conflicting priorities and tight timeframes.
  • Demonstrated experience in negotiation and engagement with key stakeholders at all levels through a collaborative approach.
  • Demonstrated understanding and application of quality frameworks, client-centred clinical service delivery, best practice applications, and models of integrated care.
  • Demonstrated ability to develop, evaluate, and report on program/service efficiencies and develop mitigation strategies where appropriate.
  • Australian C-Class driver's license (including P1 or P2).
