
Support Coordinator Specialist

We are seeking a highly skilled Social Worker to join our team in delivering exceptional support services to individuals with disability.
This is an exciting opportunity for a motivated and compassionate professional to make a positive impact in the lives of our clients.
The successful candidate will have excellent case management experience, strong interpersonal skills, and the ability to work collaboratively with other team members.
Key responsibilities include:
- Providing specialist support coordination to NDIS participants
- Assisting clients to achieve their goals outlined in their NDIS plans
- Engaging with providers to understand and respond to risk and complexity
- Reporting to NDIS and stakeholders on a timely basis
The ideal candidate will possess a strong understanding of risk factors experienced by participants with high-risk and complex needs.
They will be able to prioritize work in a busy environment while adhering to reporting requirements and deadlines.
