Our Mission

We support people to build capacity and reach meaningful outcomes through collaborative, therapeutic care. We deliver this through the provision of Allied Health, Youth Residential, Support Coordination, Lifestyle Support and Out of Home Care services across Southeast Queensland

The Role

We are searching for our next Specialist Support Coordinator—a driven, adaptable, and compassionate professional ready to deliver exceptional NDIS support coordination services. You will work closely with participants to navigate challenges, maximize their funding, and build capacity for long-term success.

Key Responsibilities
  • Provide Specialist Support Coordination for NDIS participants with complex needs.
  • Identify and address barriers to achieving participants' goals.
  • Collaborate proactively with service providers to ensure holistic support.
  • Maintain up-to-date knowledge of NDIS policies, local services, and relevant legislation.
  • Prepare timely and accurate reports for NDIS stakeholders.
  • Advocate for participants and ensure their budgets are maximized effectively.
  • Conduct regular stakeholder meetings and follow-ups.
  • Engage in professional development and ethical practice.
Welcoming Social Workers & Case Managers

We strongly encourage applications from professionals with backgrounds in Social Work and Case Management. Your experience in advocacy, psychosocial support, and systems navigation is highly valued in this role. You'll bring a trauma-informed, strengths-based approach that aligns with our commitment to person-centered care.

About You

To excel in this role, you will bring:

  • A formal qualification in Allied Health or a related field (Occupational Therapy, Social Work, Psychology, Education).
  • Experience as a Specialist Support Coordinator (highly regarded but not essential).
  • Background in Case Management (preferred).
  • Exceptional communication, collaboration, and interpersonal skills.
  • Ability to work autonomously while contributing to a supportive team.
  • Strong problem-solving skills and initiative in a dynamic environment.
  • Proficiency in service development, evaluation, and reporting.
  • QLD Australian C Class Driver's Licence with comprehensive insurance.
  • NDIS Worker Screening Check.
  • Working with Children Card (Blue Card).
  • Current First Aid & CPR certificates.
