Job Description

We are seeking a dedicated professional to deliver exceptional support coordination services for NDIS participants with complex needs. You will work closely with participants to navigate challenges, maximize their funding, and build capacity for long-term success.

The Role
  • Provide specialist support coordination for NDIS participants with complex needs.
  • Identify and address barriers to achieving participants' goals.
  • Collaborate proactively with service providers to ensure holistic support.
  • Maintain up-to-date knowledge of NDIS policies, local services, and relevant legislation.
  • Prepare timely and accurate reports for NDIS stakeholders.
  • Advocate for participants and ensure their budgets are maximized effectively.
  • Conduct regular stakeholder meetings and follow-ups.
  • Engage in professional development and ethical practice.
Key Qualifications

To excel in this role, you will bring:

  • A formal qualification in Allied Health or a related field (Occupational Therapy, Social Work, Psychology, Education).
  • Experience as a Specialist Support Coordinator (highly regarded but not essential).
  • Background in Case Management (preferred).
  • Exceptional communication, collaboration, and interpersonal skills.
  • Ability to work autonomously while contributing to a supportive team.
  • Strong problem-solving skills and initiative in a dynamic environment.
  • Proficiency in service development, evaluation, and reporting.
  • QLD Australian C Class Driver's Licence with comprehensive insurance.
  • NDIS Worker Screening Check.
  • Working with Children Card (Blue Card).
  • Current First Aid & CPR certificates.
