Our Mission

We provide therapeutic care and support people to build capacity and reach meaningful outcomes through collaborative care. We deliver this through the provision of Allied Health, Youth Residential, Support Coordination, Lifestyle Support and Out of Home Care services across Southeast Queensland.

The Role

As a Specialist Support Coordinator, you will work closely with participants to navigate challenges, maximize their funding, and build capacity for long-term success. You will provide specialist support coordination for NDIS participants with complex needs, identify and address barriers to achieving participants' goals, collaborate proactively with service providers to ensure holistic support, maintain up-to-date knowledge of NDIS policies, local services, and relevant legislation, prepare timely and accurate reports for NDIS stakeholders, advocate for participants and ensure their budgets are maximized effectively, conduct regular stakeholder meetings and follow-ups, and engage in professional development and ethical practice.

Required Skills and Qualifications
  • A formal qualification in Allied Health or a related field (Occupational Therapy, Social Work, Psychology, Education).
  • Experience as a Specialist Support Coordinator is highly regarded but not essential.
  • Background in Case Management is preferred.
  • Exceptional communication, collaboration, and interpersonal skills.
  • Ability to work autonomously while contributing to a supportive team.
  • Strong problem-solving skills and initiative in a dynamic environment.
  • Proficiency in service development, evaluation, and reporting.
