Job Description

This is an exciting opportunity to join our dynamic team as a Senior Clinical Coordinator. In this role, you will be responsible for coordinating care and providing clinical guidance to teams comprising Service Managers, Team Leaders, Registered Nurses, and Disability Support Workers. Your primary focus will be on ensuring the provision of high-quality, best-practice clinical care to our residents and participants.

Main Responsibilities:

  • Develop and implement care plans that meet the individual needs of our residents and participants.
  • Provide clinical advice and guidance to care teams to ensure the delivery of safe and effective care.
  • Assist in identifying service gaps and improvements and lead ongoing continuous improvement initiatives to achieve clinical best practice.
  • Communicate effectively with residents, participants, and care teams to build strong relationships and ensure seamless care coordination.
  • Develop and deliver training programs to enhance the skills and knowledge of care teams.
  • Promote a culture of service delivery that supports the choice and independence of our residents and participants.

Required Skills and Qualifications:

  • Current unrestricted Registered Nurse Registration as approved by the Australian Nursing and Midwifery Board.
  • Current NDIS Worker Screening Check.
  • NDIS Mandatory Worker Orientation Certificate of Completion.
  • Working with Children Check (WWCC) clearance within the last five years.
  • COVID-19 Infection Control Certificate.
  • Current COVID-19 vaccination certificates.
  • First Aid, CPR, and Anaphylaxis Training.
