About this Role

Are you a compassionate professional with a passion for empowering individuals and fostering positive change?

We are seeking a skilled Therapeutic Practitioner to join our team, where you will have the opportunity to make a meaningful difference in the lives of others.

Key Responsibilities:

  • Collaborate with care teams to assess individual needs and develop tailored support strategies.
  • Provide secondary consults to Key Workers, leveraging your expertise to inform mental health plans.
  • Cultivate strong relationships with people, facilitating goal-setting, skill-building, and independence.
  • Contribute to an environment that promotes healing-oriented practices and proactively responds to young people's needs.
  • Support individuals in building new networks and accessing specialized mental health services.
  • Maintain connections with local public, private, and non-government services.
  • Ensure timely completion of assessments and reports.
  • Engage in ongoing training and professional development.
  • Participate in meetings, debriefing, supervision, training, and forums.

Requirements:

  • Bachelor's Degree in Psychology or recognized Social Work degree with eligibility for relevant membership.
  • Demonstrated experience working within case management frameworks with complex support needs.
  • Knowledge and understanding of the Mental Health and Wellbeing Act 2022.
  • Strong passion for working with people and ability to engage actively and assertively with complex needs.
  • Experience in providing supportive counselling and case work for people with complex needs, including mental health and drug and alcohol issues.
  • Understanding or experience working with people at risk of homelessness, including knowledge of trauma-informed approaches.
  • Ability to exercise judgement and initiative when procedures are not clearly defined.
  • Proficiency in planning and report writing independently, and coordination with other stakeholders.
  • Capacity to work effectively independently and as part of a multidisciplinary team.
