Key Responsibilities:

- You will collaborate with medical specialists, treatment providers and allied health professionals to develop individualized return-to-work plans that cater to the unique needs of each ADF member.

- Conduct thorough assessments and site reviews to monitor progress and make adjustments as needed to ensure optimal outcomes.
- ">
- Implement and oversee rehabilitation plans, providing personalized support and guidance to help individuals achieve their goals.
Essential Qualifications and Skills:
- ">
- Current unrestricted registration or membership with the relevant professional body as a Registered Nurse is mandatory.
- ">
- Possess tertiary qualifications in Nursing and a strong understanding of case management principles.
- ">
- Experience working in a similar field with full registration would be highly advantageous.
- ">
- Proficiency in professional documentation and report writing is essential for this role.
- ">
- A minimum of 12 months of experience working in your profession with full registration is required.
- ">
- The ability to effectively engage with multidisciplinary teams to drive collaboration and achieve shared goals.
