Our Homelessness Stream provides high-quality services to support individuals and families experiencing homelessness, including case management, accommodation, financial assistance, and referrals to specialist services.

About the Role

We are seeking a skilled Case Manager to join our team in Spring Hill, QLD. As a Case Manager, you will provide person-centered support to individuals in achieving housing stability, social inclusion, and self-mastery.

You will work effectively with other team members to deliver a comprehensive service that reflects the values and mission of The Salvation Army.

Your Key Responsibilities
  • Establish and maintain relationships with individuals accessing our service, promoting self-determination and participation
  • Provide case management support according to TSA case management models and standards of practice
  • Conduct thorough assessments, including needs, risks, and vulnerabilities
  • Ongoing assessment of individual presenting issues, vulnerabilities, and needs from initial intake to exit
  • Develop individual case plans in collaboration with the individual, incorporating interventions to address needs and goals
