
Clinical Psychosocial Intervention Specialist

We are seeking a highly skilled and compassionate Clinical Psychosocial Intervention Specialist to join our team. This is an exciting opportunity for you to make a real impact in the lives of patients facing complex physical, mental health, and social challenges.
The successful candidate will be responsible for conducting comprehensive assessments, co-designing care plans, and providing assertive follow-up, including home visits, to support recovery and long-term wellbeing. You will work within a medically led multidisciplinary team to help shape clinical procedures, strengthen referral pathways across community services, and contribute to team operations and reviews.

Key Responsibilities- Conduct comprehensive assessments to identify patient needs and develop personalized care plans.
- Provide assertive follow-up, including home visits, to support patient recovery and wellbeing.
- Work collaboratively with a medically led multidisciplinary team to develop and implement effective clinical procedures.
- Strengthen referral pathways across community services to ensure seamless patient care.
- Contribute to team operations and reviews to continuously improve patient outcomes.
- Tertiary qualifications in Social Work or Occupational Therapy or Rehabilitation Counsellor.
- Demonstrated clinical skills in assessment, planning, and evaluation of psychosocial interventions with adult patients.
- Demonstrated experience with advanced reasoning in patient assessments of cognitive capacity and complex issues.
- Demonstrated commitment to professional and ethical boundaries in consultation with a medically led multidisciplinary treatment team.
- Knowledge of the social and emotional needs of patients with drug and alcohol issues.
