Senior Clinical Counselor

The Assertive Community Management team is seeking a dedicated clinician to deliver frontline services to patients facing complex physical, mental health, and social challenges.
This role involves conducting comprehensive assessments, co-designing care plans, and providing assertive follow-up support to promote recovery and long-term wellbeing.
Key Responsibilities:- Deliver high-quality, client-centered care to individuals navigating drug and alcohol dependency
- Conduct thorough assessments and develop tailored care plans in collaboration with the treatment team
- Provide regular follow-up support, including home visits, to foster ongoing recovery and wellbeing
- Cultivate strong partnerships with community services to enhance referral pathways and improve patient outcomes
- Tertiary qualifications in Social Work, Occupational Therapy, or Rehabilitation Counselling
- Current membership or eligibility with the Australian Association of Social Workers or AHPRA registration as an Occupational Therapist
- Demonstrated expertise in clinical assessment, planning, and evaluation of psychosocial interventions for adult patients
- Experience working with patients who have experienced trauma, grief, loss, domestic violence, and child protection
