Assertive Community Management Opportunity

This role offers a unique chance to work with a multidisciplinary team delivering frontline services to individuals facing complex challenges.

The successful candidate will be responsible for conducting comprehensive assessments, co-designing care plans, and providing assertive follow-up support to promote recovery and long-term wellbeing.

Key responsibilities include:

  • Conducting thorough assessments and developing personalized care plans.
  • Providing home visits and assertive follow-up support to patients.
  • Contributing to the development of clinical procedures and referral pathways.
  • Building relationships with patients, families, and carers to promote holistic care and support.
  • Participating in quality improvement initiatives and contributing to policy development.

For this role, we require a tertiary qualification in Social Work or Occupational Therapy or Rehabilitation Counselling, as well as demonstrated clinical skills in assessment, planning, and evaluation of psychosocial interventions.

The ideal candidate will have experience working with patients facing drug and alcohol issues, including trauma, grief, and loss, domestic and family violence, and child protection.
