
Support Professionals Wanted

As a Support Worker, you will be responsible for delivering personalized care and support to assist older adult clients maintain and grow their independence. This may include assisting with daily living tasks, such as personal care, household chores, and meal preparation, as well as providing companionship and emotional support.
You will work closely with clients to understand their needs and preferences, and develop individualized service plans to meet their unique requirements. You will also be responsible for implementing healthcare plans, positive behavior support strategies, and other interventions to promote well-being and quality of life.

Key Responsibilities:- Empowering Independence: Delivering personalized care and support to help older adult clients maintain and grow their independence.
- Making a Positive Impact: Implementing individual service plans, healthcare plans, and positive behavior support strategies to promote well-being and quality of life.
- Creating Connection: Providing companionship, engaging conversation, and emotional support while assisting with personal care needs in the comfort of the client's home.
- Helping with Travel: Assisting clients with transportation to important appointments, ensuring they stay connected and supported in their healthcare journey.
- A minimum Certificate III qualification in Aged Care or Individual Support (must have completed aged care components).
- Experience working with older adults.
- The ability to follow and implement individual service plans, healthcare, and positive behavior support plans.
- A valid driver's licence.
