Care Manager Job Opportunity

We are seeking a skilled and experienced Care Manager to lead our team in delivering exceptional care to residents.

About the Role
  • Manage the care of residents, ensuring safe and high-quality care aligned with Aged Care Quality Standards, regulatory requirements, and professional practice guidelines.
  • Educate, mentor, and support care staff to deliver a high level of quality care.
  • Support the admission of new residents, including assessments, care planning, and reviews.
  • Manage complex healthcare issues, such as wound care, infection control, pain management, falls, and continence issues.
  • Consult with residents, their families, allied health, and medical practitioners.
  • Lead the accurate and timely completion of reporting and documentation, in line with relevant legislation, regulatory requirements, professional standards, policies, and procedures.
Key Responsibilities
  • Deliver person-centred care to residents, focusing on their individual needs and preferences.
  • Maintain accurate and up-to-date records of resident care, including assessments, care plans, and progress notes.
  • Collaborate with multidisciplinary teams, including nursing, allied health, and medical professionals, to ensure comprehensive and coordinated care.
  • Identify and address any concerns or issues related to resident care, escalating them to senior staff as needed.
Requirements
  • At least two years of experience in a care management role in residential aged care.
  • Passionate about delivering person-centred care.
  • Excellent communication skills (written, verbal, and interpersonal).
  • Ability to work autonomously and as part of a team.
  • Registration with AHPRA as a registered nurse is essential.
  • Hold and maintain NDIS Worker Checks.
