Home Care Professional Role

We are seeking an experienced home care professional to join our team in Queensland, Australia.

This role involves holding clinical oversight and delivering care management to enable older people to live safely and independently at home. The home care professional works within a rights-based, person-centred framework, collaborating with participants, their supporters, and interdisciplinary teams to coordinate tailored support.

Key Responsibilities:
  • Develop individualised care plans within agreed timeframes
  • Incorporate participant goals, service preferences, cultural needs, risk factors, and environmental requirements into care plans and delivery
  • Establish and update care plans and funding requirements, ensuring participants understand their available services
  • Work as part of a team-based model, providing clinical care management and guidance to multidisciplinary teams
About You:
  • Bachelor's degree in nursing, social work, allied health, or similar
  • Demonstrated experience in care coordination, case management, or similar roles in aged care, health, or disability services
  • Solid understanding of the Support at Home Program, Quality Standards, and aged care rights
  • Competence in interpreting clinical guidance and escalating appropriately
